​These forms are required to have on file and submitted with a purchase request in order to pay many of our vendors. If a company is registered adequately in Sam.gov, they will not need to complete the 889 certifications.

1. SECNAV 5512

​This form is required for all people 18 years and older who do not have a DoW ID card. Those 16-17-year-old are also required to complete this form if they will be driving themselves onto base.
